Claudia Chernow, MD
Founder
Dr. Chernow is a double board certified psychiatrist specializing in Child and Adolescent Psychiatry and Adult Psychiatry. She has been practicing psychiatry for 43 years and opened her private practice in 1988 to provide thoughtful, high quality care in a personalized setting.
Dr. Chernow completed her general psychiatry residency at The Institute of the Pennsylvania Hospital in Philadelphia and her Child and Adolescent Psychiatry fellowship at Hahnemann University Hospital, also in Philadelphia. During her training at the Institute, she served as an instructor through the University of Pennsylvania, teaching medical students.
She earned her Doctor of Medicine degree from Sidney Kimmel Medical College at Thomas Jefferson University in Philadelphia, Pennsylvania, and received her undergraduate degree from Princeton University.
Dr. Chernow is a member of the American Academy of Child and Adolescent Psychiatry and a Distinguished Life Fellow of the American Psychiatric Association.
Over many years, she has also provided school consultation services to school districts in Montgomery County, Bucks County, and Philadelphia. In this role, she may evaluate individual students and also collaborate with school teams over time to support stronger educational outcomes.

Hope Jann, PA-C
Hope Jann, PA-C, is an experienced Physician Assistant with more than two decades of clinical practice across behavioral health and primary care. She focuses on careful evaluation, medication management, and coordinating care that supports patients and families through complex medical and psychiatric needs.
Since 2015, she has worked at Seeds of Hope Partial Hospitalization Program in Exton, Pennsylvania, where she evaluates and treats patients with anorexia, bulimia, and other eating disorders, including medication management, ordering and interpreting labs and cardiac testing, and collaborating closely with the full treatment team. She also serves at Life Counseling Services in Paoli, Pennsylvania, providing psychiatric evaluation and medication management for intensive outpatient programs, including adolescent and adult patients with mental health and substance use concerns.
Hope earned her Master’s in Physician Assistant Studies from Arcadia University and her Bachelor of Science in Biology from Muhlenberg College. She is NCCPA certified, licensed in Pennsylvania, holds DEA registration, and is active in professional PA organizations.
